The Home and Community Care Program (HACC PYP) provides basic support for individuals and their carers to help them remain independent and safe at home. 

Council is not a National Disability Insurance Scheme (NDIS) provider.

The program focuses on helping individuals explore ways to live independently and remain active members of their community. It takes a holistic approach to ones needs promoting wellness, capacity building and furthermore assisting individuals to connect with appropriate supports that best suit their needs.

Who can access the service?

Support at home is available for residents with a disability under the age of 65 and Aboriginal and Torres Strait Islanders under the age of 50.

What supports are available?

• Domestic assistance includes mopping and vacuuming, cleaning the bathroom
• Delivered meals to your home or group meals at community venues
• Personal care includes assistance with showering and grooming
• Respite (carer support)
• Property maintenance includes gutter cleaning, installation of smoke alarm, non-slip solutions and additional support to maintain a safe and accessible property
• Home modifications includes installation of rails and ramps
• The Home and Community Care program also provides advocacy to support individuals to navigate the best pathways to meet their needs. For people with more complex needs, which cannot be met by mainstream HACC PYP services alone, our Intake/Triage Officer can advise and support you to link into programs that can best assist you.

Support for carers

Carers need a break too. A break can give you time to do everyday activities, look after yourself and tend to your own needs. When you are a carer, it is important to regularly take time out for yourself to rest and recharge.

Respite care provides a break for you and the person you care for through:

• respite in your home or in the community
•  assistance to link in with appropriate carer support services offered by other agencies, such as Carer Gateway
•  providing occasional support to assist with the tasks you would normally do for the person you care for, whilst you are managing your own health care needs and wellbeing
•  advocacy for you and the person you care for to be recognised and supported.

Changing Places toilets cater for the needs of adults and children with severe and profound disabilities who require specialist equipment.

Each Changing Places toilet provides: 

• a height adjustable adult-sized changing bench,
• a tracking hoist system,
• enough space (larger than standard accessible toilets),
• a safe and clean environment.

You’ll need a master key
You will need a master key which is called MLAK key. The MLAK key fits into specially designed locks allowing 24 hour a day access to Changing Places and other special facilities. Download the MLAK key request form(DOC, 38KB).

Return signed form to any of Council's Customer Service Centres in Broadmeadows, Craigieburn or Sunbury to receive your key.

Receiving the right support can sometimes be challenging, particularly for those who may have additional barriers to overcome when accessing services.

The Access & Support Officer is available to provide short term support and guidance to people from diverse and vulnerable backgrounds to access and navigate the support they need.

The Hume Access & Support Officer works predominantly with those from Aboriginal and Torres Strait Islander communities and Culturally and Linguistically Diverse backgrounds. Support can also be provided to other groups known to be vulnerable for example people who identify as LGBTIQA+, people from financial and socially disadvantaged circumstances and people who are at risk of homelessness.

Hume Libraries offer a free home library delivery service to those who are homebound or unable to visit the library due to illness, frailty or disability. Library items are selected and delivered by staff or volunteers.

This service is also available to residents of nursing homes, retirement villages and hostels.

Free computer training programs are offered at Hume Libraries and learning centres. These programs are designed to teach you the basic skills to get you started using computers, computer applications and the internet.

Net.Help

Net.Help offers an introduction to email and the Internet for adults. Sessions can be designed for individuals or groups.

Classes are run at the Hume Global Learning Centre in Broadmeadows and are conducted by trained volunteers.

Playgrounds provide wonderful opportunities for children to be physically active, interact with other children and learn skills such as cooperation, socialising and playing independently. Playgrounds with accessible equipment allow children with a disability to have fun and interact with other children.

Features include accessible swings and other play equipment, continuous pathways, accessible entries to playgrounds and parking close by. Many of the parks have play equipment for a range of ages, and other recreation opportunities nearby such as ovals, half-court basketball rings and tennis courts.

We recognise the important role of carers, and understand that they sometimes need support.

Both in-house and centre-based respite care services are available to give carers a break from their caring role. The respite care is designed to provide the carer with the necessary peace of mind to benefit from the respite care. 

Hume's home and community support services provide a range of services to eligible residents who are frail older people, people with disabilities and carers. An assessment is completed prior to a service being provided.

Emergency Respite

Emergency respite can be arranged by calling Carerlinks North on 1800 059 059. 

Community Respite

Community respite service provides an opportunity for the care recipient to spend time in the community while giving the carer a break from the caring role. Duties completed in the community are focused on the interests of the client e.g. a person with an interest in gardening may enjoy time at a park, garden or nursery. For community respite there is an additional charge for travel over 10km from the home.

In-Home Respite

In-home respite service provides an opportunity for the carer to have a break from the caring role. It is designed so that the carer is able to access the community knowing the person they care for is being supervised in the home. Duties completed during respite care are focused on the interests of the recipient. Tasks may be reading, looking at books, talking, supervising clients with light gardening and light cooking.

The Recharge Scheme invites businesses to provide a power point for people to recharge the batteries of their electric wheelchair or scooter.

The batteries on electric wheelchairs and scooters last only a few hours, and so having many free accessible Recharge locations across the Hume City area allows people to travel for longer and be more active in their community.

Businesses involved in the Recharge scheme will have a Recharge sticker displayed on their shop window, letting people know that they are a Recharge location.

Anyone with an intellectual disability is invited to join in our hip-hop dance classes.  The class is based on hip-hop moves to music, but encourages exercise and socialisation.

SwimAbility provides swimming lessons for children and adults with autism, auditory processing delay, communication difficulties, sensory processing difficulties and developmental delay.

Classes are for all ages from 6 months old through to adults, they include:

• Free assessment to determine your child's suitable entry level
• Weekly 30 minute classes
• Small class sizes
• Swim and Survive certificates issued on progression of levels.

Our swimming instructors are provided with training and resources to create a more supportive learning environment for children with specific learning needs and their families. 

Inclusion swimming lessons are designed to develop skills in water safety, water familiarisation, breathe control and swimming skills such as kicking, floating and arm movements.

Parents/carers are required in the water to assist with the Inclusion Swimming Program.

Cost

• Swim and Play lessons are $29.95 per fortnight, payable via direct debit.
• Discounts available for families with more than one child.
• Sign up with direct debit options and receive a free swimming gift pack and free access to all Hume City Council pools.
