Aged Care Reforms and Our Home Care Package Program

From 1 July 2025, Australia’s aged care system will undergo an important transition to the new Support At Home (SAH) Program to give older Australians greater choice, flexibility, and control. At Hume City Council, we’re fully prepared to ensure a smooth transition from our current Home Care Package (HCP) Program to the SAH Program while continuing to deliver exceptional care we are known for. 

What You Need to Know

For existing customers: Your service will continue without major changes. You will experience some changes in the way your package is handled, but you will not be disadvantaged.

About the reforms: The new SAH program goes in tandem with the Aged Care Act 2024, that was passed on 25 Nov 2024. With the Act and the new Program, the Commonwealth aims to improve access and quality of care for all Australians.

FAQs for Support At Home Program 

What is the Support At Home Program?

Support At Home offers a wide range of support so you can continue to live well at home. Based on your aged care assessment and eligibility, you will have access to an approved list of services. You will be supported to choose the mix of services (i.e., Clinical Care, Independence, and Everyday Living services) you can access within your budget. 

When does Support At Home start, and what does it replace?

It will start on 1 July 2025, and replaces the Home Care Packages Program under Hume City Council. The Commonwealth Home Support Programme (CHSP) will continue at Hume, and only transition to Support at Home no earlier than 1 July 2027. 

How do I enter the Support At Home Program?

The first step to access government-funded aged care services is to check your eligibility for an assessment. You can do this online at My Aged Care, or by calling 1800 200 422. 

If your application is successful, you will be referred for an assessment. Assessments are done in person, at your own home. 

When assessed as eligible for the Support at Home program, you will receive a notice of decision and an individual support plan to share with your provider. If you Choose Hume City Council as your provider, we’ll get to know you during a home visit, to discuss your immediate and long-term needs, and how your funding is spent.  

I am an existing Home Care Package client with Hume City Council, what happens to me under the Support At Home Program?

What are the key changes for Home Care Packages (HCP) Program recipients transitioning to Support at Home from 1 July 2025?

Quarterly budgets, where recipients who have not spent their budget within the quarter will be able to accrue a maximum of $1,000 or 10% of their quarterly budget, (whichever is higher,) from one quarter to the next.  

A defined service list, with all services specified with clear inclusions and exclusions.
