Assertive Outreach Support for Young People in Hume

Hume City Council has engaged Youth Projects to deliver assertive outreach support to young people in Hume aged 12 – 25 years.  YHOP is an assertive outreach service that engages and connects at-risk young people to individualised housing, mental health, education, training, employment and crisis supports across Hume.

YHOP works with young people for up to 6-months to overcome barriers and provide tailored support based on the outcomes, goals and aspirations of each young person.
